Dynamic forms provide a simple way for non-technical users to take paper-based forms and quickly build accessible, interactive, electric forms and collect data from on and off-campus users.  It is best to schedule a meeting with our Dynamic Forms team to determine if your current document/form is a good fit for conversion to Dynamic Forms.  Please contact Dwight Turner to set up this meeting.

Will Dynamic Forms work for me?

In most cases, Dynamic forms will work for the collection of data with the exception of certain highly sensitive data.

We allow the collection of the following data elements through Dynamic Forms:

  • Wet signatures
    • Any electronic signature that is contractual in nature
  • HIPPA-protected health information
  • FERPA-protected personal information

We do not allow the collection of the following data elements through Dynamic Forms:

  • Social Security Numbers

Dynamic Forms can also query data directly from Banner and use that data to automatically populate elements on your forms - for instance, a student's name, address, etc. can populate automatically once a Banner ID is entered into the form.  

Dynamic Forms vs. DocuSign

DocuSign is a Cloud-based software solution that allows the electronic signing, sending, and managing of documents.  There is a fee associated with DocuSign (users pay a fee for each document sent/signed); Dynamic Forms is a free software with no associated costs per document.

Use Dynamic Forms When:

  • You need a document with workflows (multiple stakeholders/approvals)
  • You require an electronic signature that is contractual OR an electronic acknowledgement that is non-contractual
  • You need to read data from and/or write data to Banner

Use DocuSign when:

  • You require API integration with third-party software
  • If you are signing a contract on behalf of the University

How do I request a form be converted to Dynamic Forms?

Requesting a form in Dynamic Forms is an easy process:

  1. Start by taking a look at the "Will Dynamic Forms Work For Me?" section and make sure that the data elements you wish to collect are allowed
    1. If you do have elements that Dynamic Forms cannot collect, we have other options available; contact us, and we can discuss alternatives
  2. Review all of the data elements in your form and the workflow steps that you would like for it to follow - we recommend simplifying your workflow as much as possible
  3. Once you are ready to request your form, submit a support request using the Dynamic Forms Request

How do I view my Dynamic Forms requests?

Log into Dynamic Forms  and click "My Forms" in the top right.
